Clinical / Consulting Services

PharmaCare offers consulting services by its team of professional pharmacists. The pharmacist identifies potential drug interactions, while ensuring each medication that is currently being prescribed matches the diagnosis. Additionally, the pharmacist makes sure that current doses are appropriate for the resident. It is also the role of the pharmacist to make recommendations after careful analysis of the resident's disease state regarding the current therapy and cost of medications.

There is an experienced LTC nurse who provides quality assurance reviews for all facilities. Audits are performed on the patient care level, and physician order summaries are reviewed to insure proper care and procedure by all staff.

Clinical Services

PharmaCare’s Medical Records Department prints and maintains physician orders, medical administration records (MAR), and treatment administration records (TAR). Their role is to ensure the integrity of the recording process and provide appropriate documentation for each resident.
