Patient Rights & Responsibilities

No matter what type of coverage you have, you carry certain rights and responsibilities. As a patient of PharmaCare, you have the right to all of the following:

  • Be treated with dignity and respect at all times
  • Be protected from discrimination
  • Have access to your pharmacist
  • Have the right to competent counseling from the pharmacist to help understand your medications and how to use them correctly
  • Have your questions about your prescription medications answered related to specific drug therapy, possible adverse side effects and drug interactions
  • Have access to your pharmacy profile and prescription information
  • Have assistance with decisions concerning your prescription drug coverage including insurance limitations, payment, and coverage decisions
  • File complaints (grievances), including complaints about the quality of your care
  • Have your personal and health information kept private
  • Report medication errors immediately to the pharmacist as well as being an active participant in the prevenetion of medication errors by reviewing our “Medication Safety Guide”

In addition to these rights, as a patient of PharmaCare you have the following responsibilities:

  • Provide the pharmacy with a complete and accurate health history
  • Provide the pharmacy with all requested insurance and personal information
  • Notify the pharmacy of any changes in insurance or personal information
  • Sign the required agreements and releases for services and insurance billing requirements
  • Participate in your care plan by asking questions and following the directions provided by your pharmacist
  • Provide a safe home environment to help prevent medication errors and diversions
  • Accept the consequences for any refusal of treatment or choices for noncompliance
  • Notify the pharmacy of any problems or dissatisfaction with your care

Code of Ethics

As healthcare professionals, pharmacists assist individuals in making decisions on how to best use the medications they are prescribed. All PharmaCare employees have a fundamental role and responsibility to follow this code:

  1. Respect the relationship between patient and pharmacist.
  2. Promote the good of every patient in a caring, compassionate and confidential manner.
  3. Respect the autonomy and dignity of each patient.
  4. Act with honesty and integrity in professional relationships.
  5. Maintain professional competence.
  6. Respect the values and abilities of colleagues and other health professionals.
  7. Serve individual, community and societal needs.
  8. Seeks justice in the distribution of health resources.

In accordance with the Health Insurance Portability and Accountability Act of 1996 (HIPAA). The PharmaCare Network pledges to protect your health information as required by law. The Privacy Notice available for download below will tell you (1) how we will use and disclose your “protected” health information (PHI) and (2) how you can exercise certain individual rights related to your “protected” health information (PHI).
